HPV相關癌症中「口咽癌」已超越子宮頸癌! 兩性應共同預防HPV 遠離HPV感染相關風險
大多數的HPV感染會自行清除,若長期持續感染HPV且無法自行清除,可能導致HPV相關疾病。HPV感染原因多 單一性伴侶也應注意HPV相關癌症中「口咽癌」已超越子宮頸癌(註1)。過去民眾只會聯想到子宮頸癌的人類乳突病毒(HPV),實際上也可能會引起頭頸癌、肛門癌等疾病(註2)。根據統計男性罹患頭頸癌的風險較高(註3),不可不慎!台中榮民總醫院耳鼻喉頭頸部主任王仲祺醫師分享,人的一生有高達8成機率感染HPV(註4) ,主要經由性接觸傳染,但因其具高度傳染性,皮膚、黏膜等接觸到HPV就有感染風險(註5),研究指出就算只有單一個性伴侶,感染HPV的機率仍有6成,若在一生中累積3至6位的性伴侶,感染HPV的風險更高達9成(註4)。台灣每50人就有將近1人口咽部感染HPV!台灣近年因感染HPV而導致的口咽癌呈上升趨勢(註6),口咽癌在美國則已超越子宮頸癌成為最常見的HPV相關癌症(註1),好萊塢巨星麥克道格拉斯(Michael Douglas)就曾因罹患第4期咽喉癌差點喪命,他受訪時曾表示他罹患咽喉癌不是因爲吸菸或過量飲酒,而是因為感染HPV所導致。2023年本土研究發現,台灣將近每50人中就有1人口咽部感染HPV,當中男性口咽部HPV感染盛行率更是女性的1.3倍 (註7)。王仲祺醫師補充,除了菸、酒、檳榔外,不分男女皆有可能因HPV持續感染而罹患頭頸癌(註8)。研究指出,和女性相比,男性感染 HPV 後較不容易自然產生抗體,長期感染且無法自行清除時致癌風險較高(註9)。即便在感染初期沒有明顯症狀(註5),也可能在不知不覺中傳染給親密朋友。頭頸癌患者多為中年男性,常因工作延誤治療王仲祺醫師分享診間觀察,曾發現有病患因頭頸癌早期症狀不明顯,而延誤最佳治療時機(註5)。經治療後,病患的外貌可能受影響,甚至也造成了病患生活上的不便性,像是難以說話、吞嚥等症狀,輕則嘴巴乾燥、重則喉嚨壞死。這些生活上的不便性也可能造成病患容易有憂鬱、社交畏懼及抗拒親密關係的情況發生(註10,11), 。且這些患者大多為40至50歲的中年男性族群(註12),這些中年男性族群往往都是家中的經濟支柱,常因工作而延誤治療時機,未來反而會帶來更龐大的醫療負擔。為了避免後續治療對家庭帶來更大的負擔,因此更需要有HPV防範意識。醫籲HPV三訣竅 民眾應及早發現、及早治療頭頸癌沒有常規篩檢工具,因此不易早期發現(註13),為了避免病患們在手術治療後可能導致的生心理上的不適感,高風險人群可優先透過免費的口腔癌篩檢工具來預防頭頸癌,40歲以上的民眾應定期接受成人健檢服務(註14),盡可能及早發現、及早治療,保有較好的生活品質亦可降低龐大醫療費用。王仲祺醫師最後建議民眾不分性別應及早採取行動預防,除了戒菸、酒、檳榔外,更應注意安全性行為、依醫囑接種HPV疫苗(註13)。和所有疫苗一樣,HPV疫苗並非用於治療且可能無法對所有的疫苗接種者都產生保護作用。常發生的不良反應為注射部位搔癢、腫塊、溫熱感等,或頭痛、發燒等全身性不良反應,通常輕微且短暫。其他詳細資訊,請諮詢醫療專業人員。參考資料註1:Van Dyne EA, Henley SJ, Saraiya M, Thomas CC, Markowitz LE, Benard VB. 醬油、醋、番茄醬、蜂蜜要不要放冰箱?專家曝調味料最佳保存方式
番茄醬是家中必備的調味料之一,可以拿來煮茄汁煮義大利麵、作為雞塊跟薯條的沾醬。究竟番茄醬要不要放入冰箱冷藏保存?來聽聽美國專家怎麼說。番茄醬要不要放冰箱保存?根據美國有線電視新聞網(CNN)報導,美國知名調味料品牌卡夫亨氏食品公司(Kraft Heinz)近日在twitter發布一條推文,重申番茄醬應該要冰進冰箱,讓「番茄醬要不要冰」的議題引起熱烈討論。2023年6月27日,亨氏美國食品公司再度發文,僅用五個字「Ketchup. goes. in. the. fridge!!!」(番茄醬要冰進冰箱),就引發了一場關於如何儲存番茄醬的爭論,該推文至今吸引了超過517萬人次瀏覽。FYI: Ketchup. goes. in. the. fridge!!!— Heinz (@HeinzUK) 2023年6月27日而一天後(6/28),該公司通過twitter的民意調查,詢問大眾番茄醬該如何保存:放在冰箱冷藏/在室溫的櫥櫃中。調查結果顯示,13,000多張選票中,有63.2%的人選擇了「番茄醬要放冰箱」,36.8%的人選擇「番茄醬要放櫥櫃」。對於此貼文的票選結果,不喜歡冰冷口感番茄醬的部分網友表示,一般餐廳也都把番茄醬放在室溫桌子上;但其他網友不明白爭論的必要性,聲稱番茄醬瓶一旦打開,就應該放在冰箱裡。Where do you keep yours? 權威期刊研究稱中國研發VV116比Paxlovid更安全?教授這麼說
新英格蘭醫學期刊前天(2022-12-28)發表VV116 versus Nirmatrelvir–Ritonavir for Oral Treatment of Covid-19(VV116 對比Nirmatrelvir–Ritonavir用於 Covid-19 的口服治療)。這是一項三期臨床試驗。VV116是中國研發的口服瑞德西韋(Remdesivir),而Nirmatrelvir–Ritonavir則是目前首選的新冠藥物,也就是輝瑞的Paxlovid。所以這項研究就是要看VV116是否能勝過Paxlovid,而它的結論是:「在有進展風險的輕度至中度 Covid-19 成人患者中,VV116 在持續臨床恢復時間方面不劣於 nirmatrelvir-ritonavir,且安全性問題較少。」我們先來看為什麼「安全性問題較少」。我在新冠口服藥:輝瑞與默克,優劣與禁忌(上)這篇文章裡有說,Nirmatrelvir是對抗新冠病毒的M蛋白酶,能抑制病毒的複製,而Ritonavir則是對抗人體裡的CYP3A4。CYP3A4是一種酶,主要存在於肝臟和小腸。它可以氧化外源有機小分子,如毒素和藥物(例如Nirmatrelvir),促使其排出體外。在同時服用Ritonavir的情況下,Nirmatrelvir的血中濃度可以增加大約8倍。也就是說,因為有了Ritonavir,才使得Nirmatrelvir能成為有高效力的抗新冠特效藥。可是,由於Ritonavir 會抑制CYP3A4,所以它當然也會影響非常多藥物在體內的代謝。美國國家健康研究院就列舉了104種會受到影響的藥,而這些藥都是很多人平常都在服用的(例如他汀類)。這就是為什麼,相對於nirmatrelvir-ritonavir來說,VV116的安全性問題較少。VV116主要是由中國的五家學術機構和兩家公司共同研發。五家學術機構是:中國科學院上海藥物研究所、中國科學院武漢病毒研究所、中國科學院新疆理化技術研究所、中國科學院中亞藥物研發中心/中烏醫藥科技城、臨港實驗室。兩家公司是:旺山旺水生物醫藥公司和君實生物科學公司。第一篇有關VV116的論文是2021-9-28發表的Design and development of an oral remdesivir derivative VV116 against SARS-CoV-2(針對 SARS-CoV-2 的口服瑞德西韋衍生物 VV116 的設計和開發)。我在武漢肺炎:試驗新藥似乎有效這篇文章有說,瑞德西韋是美國的吉利德公司原本為了治療伊波拉病毒而研發出來的藥,但後來放棄。它在2020年1月被用來試驗性地治療美國第一位被證實感染新冠肺炎的病人。它是靜脈點滴針劑,而療程是5到10天,所以對病患而言是相當不方便。從上面那篇論文的標題就可看出VV116是口服藥,但嚴格來說,VV116並不是瑞德西韋的衍生物。事實上,論文裡面有清清楚楚地說,VV116是GS-441524的衍生物。GS-441524也是吉利德公司研發出來的藥,但一直沒有商業化,所以到目前都還只是用GS-441524來作為代號。GS-441524是口服藥,而瑞德西韋則是靜脈點滴藥。事實上吉利德公司是先研發出GS-441524(2009年獲得專利),然後才將它修改成瑞德西韋(2017年獲得專利)。那,為什麼吉利德公司會捨棄較方便的GS-441524,而選擇較麻煩的瑞德西韋來商業化?目前有兩種說法。第一,由於GS-441524的專利技術有許多別家公司研發的先例,所以可能會經不起這些公司的挑戰而賺不了錢。請看GS-441524 for COVID19 not pursued by GILEAD(吉利德不追求用GS-441524來治療COVID19 )。第二,因為GS-441524的專利會比瑞德西韋的專利早8年失效,所以GS-441524的賺錢期限也就會比瑞德西韋少了8年。請看How China beat US pharma giant Gilead in the race to create a covid antiviral pill(中國如何在創造一個抗新冠病毒藥的競賽中擊敗美國製藥巨頭吉利德)。那,也許您會問,中國的VV116就不怕被吉利德挑戰嗎?答案是,吉利德可能會挑戰,但成功的機會不大。這是因為,中國團隊是將GS-441524通過一個叫做deuteration(氘化)的步驟而衍生出VV116,而這個步驟就可繞過GS-441524的專利。(註:氘2H是氫1H的同位素)有關deuteration,我們來看一篇2014年發表的論文Deuterated drugs; where are we now?(氘化藥物; 我們現在在哪裡?)裡面的一段話:「碳-氘鍵比碳-氫要強得多。所以,在藥物分子中用氘來替代氫可以導致藥物生物學效應的有益變化,例如通過降低藥物代謝速率從而減少給藥頻率。這種替代還可以通過減少有毒代謝物的形成而具有降低毒性的作用。」吉利德是在今年一月宣布他們正在研發一個名叫GS-5245 的口服新冠肺炎藥。但直至目前這項研究還只是在一期試驗,遠遠落後已經完成三期試驗的VV116。原文:中國研發的口服瑞德西韋,吉利德錯失良機

依據嚴重程度決定治療方式 醫師詳解口腔癌症狀如何分期
「得了口腔癌,只能開下巴嗎?」手術切除是治療口腔癌最主要的方法,但根據癌症的嚴重程度,也可能會採取放射、化學藥物等治療方法,抑或是合併使用不同的療法。讓我們來認識口腔癌的治療方法吧!口腔癌的診斷依據及分期口腔癌的治療方法主要是根據臨床的診斷及分期來決定,因此在進入正題以前,先看看如何診斷口腔癌!首先,醫師會藉由理學檢查中的視診來評估口腔的症狀,如口腔黏膜顏色、質地的變化,或是出現增生物、出血等。若評估疑似有口腔癌病變,便會進一步進行內視鏡的檢查,利用侵入性的儀器更深入觀察病變結構。「視診和內視鏡檢查看起來都像是癌症病變,那就是得口腔癌了嗎?」口腔癌最重要的診斷方法為病理組織切片檢查,除了前述理學檢查及內視鏡檢查外,仍須搭配切片檢查的結果才能確定是不是癌症。另外,也會透過顎骨X光、電腦斷層或核磁共振檢查來幫助定位及分期喔!根據美國癌症聯合委員會(American Joint Committee on Cancer, AJCC),目前口腔癌的分期方法主要是根據原發腫瘤大小(T)、頸部淋巴結轉移狀況(N)、是否有遠端轉移(M)的TNM系統來決定。AJCC是一個致力於定義和推廣癌症分期明文標準的組織,目前許多國家的癌症分期都是依照這個組織的指引而訂定的。而台灣在口腔癌的診療也使用AJCC的標準,詳細分期如下[1]:第零期:原位癌腫瘤細胞侷限在口腔黏膜上皮內。第一期:病灶最長直徑小於2公分、侵犯深度小於0.5公分,且無頸部淋巴結或遠處器官轉移。第二期 有以下任何一種情形包括:1. 病灶最長直徑小於2公分、侵犯深度介於0.5至1公分,且無頸部淋巴結或遠處器官轉移。2. 病灶最長直徑介於2至4公分、侵犯深度小於1公分,且無頸部淋巴結或遠處器官轉移。第三期 有以下任何一種情形包括:1. 病灶最長直徑介於2至4公分、侵犯深度大於1公分,且無頸部淋巴結或遠處器官轉移。2. 病灶最長直徑大於4公分、侵犯深度小於1公分,且無頸部淋巴結或遠處器官轉移。3. 病灶大小、侵犯深度介於前三期內之範圍,但可觸摸到同側頸部單顆小於3公分的腫大淋巴結,無遠處器官轉移。第四期 有以下任何一種情形包括:1. 病灶大於4公分、侵犯深度大於1公分,或有周圍組織深部的侵犯。2. 頸部淋巴結轉移的數目超過一個(不論是在原發病灶的同側、對側或兩側)。3. 不論原發腫瘤大小,有對側或兩側頸部轉移或單側頸淋巴結大於3公分等。4. 不論原發腫瘤及頸部的情況,有遠處器官轉移。口腔癌的分期可以用來決定治療的方式,分成外科手術、放療、化療、其他藥物療法等,那醫師會如何選擇呢?外科治療手術切除是治療口腔癌最主要的方法,根據臨床分期的不同,切除的範圍也會有差異 [2]:第一期需切除病變處,若懷疑有潛在淋巴結侵犯的可能性,可能會加入「上頸部淋巴結切除」。第二期則一律連同病變處及上頸部淋巴結都切除。若到第三期,則做廣泛病變處切除及全頸部淋巴結廓清術。第四期則採取大範圍切除病變處及全頸部淋巴結廓清術,切除範圍可能包括臉部皮膚,或部分上、下顎骨等。手術方式包括開放式手術、內視鏡手術等。一般來說,口腔癌是透過直接目視下以開放式手術進行。針對張口困難且腫瘤較大較深的病患,則需要將下顎骨鋸開,以取得較佳的手術視野。針對適合的個案,也可以考慮使用內視鏡手術,達文西機械手臂手術進行腫瘤切除。然而,若是腫瘤太大,判斷無法以內視鏡和達文西機械手臂切除者,仍建議用開放式手術切除,因為手術的首要目標即是將腫瘤徹底切除乾淨。若病情嚴重、切除範圍廣,則需搭配顯微皮瓣重建術。頭頸部腫瘤的治療困難之一就是美觀,當腫瘤過大時,會打開下巴進行切除,而為了術後美觀,在主要的手術結束後須會同整形外科進行重建,依照患處大小,取身體其他部位的皮瓣來填補臉部的空缺。皮瓣的種類包括局部皮瓣(local flap)、遠端皮瓣(distant flap)及顯微游離皮瓣(free flap)。局部皮瓣為傷口週邊的皮瓣,遠端皮瓣為離傷口較遠的皮瓣,這兩者常用的有三角肌肌皮瓣、胸大肌肌皮瓣、斜方肌肌皮瓣、闊背肌肌皮瓣等。而游離皮瓣則是身體任一部位的皮瓣,手術時會將該部位皮膚及供應血流之血管莖(pedicle)一同取下,在口腔癌傷口處經由顯微手術接合血管,為現今較常使用的手術方式,較不受限於組織的位置及大小,常用的皮瓣如前臂橈側皮瓣、小腿腓骨皮瓣及大腿前外側皮瓣等。放射治療對於第一、二期的口腔癌病人,除了上述提及的手術切除外,也可單獨使用放射治療。而對於第三、四期的口腔癌病人,則通常需要合併手術及放射治療,以確保癌症細胞清除完全。由於放射治療會產生一定程度的副作用,造成牙齒及牙齦的傷害,也會使傷口不易癒合,因此在進行治療之前,必須要事先做口腔檢查,若有蛀牙、牙周病等情形,要預先治療,康復後才能進行放療。另外,放療也可能引起口角炎等傷口,請至牙科與口腔外科醫師治療、索取藥物。口腔癌術後可能會放置敷料或塗抹藥膏,記得清潔口腔並做好傷口照顧,以維持口腔健康。化學治療與其他藥物療法化學治療本身不能治癒口腔癌,但適用於兩個時機:1. 手術後合併放射治療,輔助清除癌細胞。2. 有遠端轉移而無法切除病灶的末期病人,可減緩疾病惡化的速度,增加病人的存活率。現今也有許多新的口腔癌療法,包括標靶藥物、免疫治療等[3]。標靶藥物如上皮生長因子受體(EGFR) 酪胺酸激酶抑制(Tyrosine kinase inhibitor),像是cetuximab及panitumumab等;免疫治療如免疫檢查點抑制劑(checkpoint inhibitors),像是anti-PD-1、anti-CTLA-4等。這些嶄新的療法在部分病人身上有顯著的療效,或許未來能解決目前口腔癌及其他癌症治療上的困境。此外,人類乳突瘤病毒(HPV)的檢測也可能有助於口腔癌的治療。根據研究[4],台灣HPV陽性的頭頸癌(包括口腔癌、咽癌、喉癌等)患者佔所有頭頸癌患者的比例逐漸增加,約有30%左右,而在國外則約佔50%。另外,研究顯示[5],HPV陽性的患者對於放射加化學治療的療效較好,故若能事先進行病毒檢測,就能提供我們治療選擇的依據。結語目前台灣的醫療技術發達,若得了口腔癌,不用擔心治不好!治療口腔癌的方式有很多,醫師會選擇最適合病人的療法,以改善病人的健康,並增進生活品質。此外,目前有部分標靶藥物納入健保給付,若傳統治療無效,也可以考慮使用。最後,在經濟方面,患者可以申請重大殘障傷病卡補助、各種社福補助等,援助病患與其家屬的生活費用。在家庭層面,若家人願意支持、陪伴治療,提供接送等援助,對於病人來說,都是最好的良藥,讓病人能夠克服接受大手術、整形重建等的心理壓力。參考資料:[1] AJCC Cancer Staging Manual, Eighth Edition (2017) published by Springer International Publishing. 日本和菓子職人水上力,被英國權威美食指南《Where Chefs Eat》評為全世界頂尖五名廚師,連以馬卡龍為傲的巴黎點心師傅,也評論水上力的和菓子令再好的馬卡龍都味同嚼蠟。水上力出身和菓子世家,從小耳濡目染在麵粉堆與和菓子模具中長大。他獨立外出學藝,在京都和名古屋的老店學藝五年,然後在東京開設「一幸庵」,一九七七年至今,七十四歲的水上力依舊不懈地開創和菓子的各種可能。和菓子在日本過去稱為「菓子」,古代日本人將天然水果或果實加工食用,如柿乾、果乾或米團等。而今日和菓子的起源則是一千多年前從唐朝傳入的「唐菓子」,根據《倭名類聚抄》記載,當時有八種以米捏製而成的「唐菓子」,分別為梅枝、桃枝、餲餬、桂心、黏臍、饆饠、鎚子、團喜以及果餅十四種。唐菓子當時單純作為祭神所用的祭品,並未成為通俗的甜點。唐菓子之後,陸續有各種點心的製作方式傳入日本,增加和菓子的種類。鎌倉時代,從宋朝學法返國的僧人帶進羊羹、饅頭類食品以及茶苗,也帶回「喝茶配茶點」的習慣,逐漸形成日本以和菓子搭配茶道的文化。經過長年的調整,和菓子逐漸發展為日本的特色甜點,除了材料與造型的變化外,大小也從原來的掌心大變為一口可食的尺寸。由於葡萄牙人將西方特色的點心帶入日本,為凸顯與歐洲糕點的區別,明治時代正式將日本傳統的「菓子」稱為「和菓子」,將外來的甜點稱為「洋菓子」,以彰顯日本獨特的糕點文化。由於和菓子向來是搭配茶道使用的點心,味道多半較為甜膩,但水上力卻不這樣想,他認為和菓子應具有更多的美學意義,可以發展為餐桌上的主角。水上力將和菓子發展為不依附抹茶而存在的餐點,不僅結合四季變化做出美不勝收的和菓子,更可結合西方繪畫、建築藝術創作日本風味的和菓子,更將和菓子的調味進行修正,使之成為獨特的存在,而非附屬於茶道,現代和菓子已然超越唐宋傳入日本時的意涵,成為日本文化的一環。如同許多日本職人與藝術家,水上力的人生持續工作與創作,沒有追求退休享受人生的思考,一幸庵的水上力如此、浮世繪的葛飾北齋如此、數寄屋橋次郎本店的小野二郎亦是如此。在他們眼中工作與生活融為一體,追求極致的藝術,而此等嚴肅的追求形塑了生活的一切。雖然不是所有人都必須如同這幾位一般,確實放下光環並找到生命的新價值是必要的轉換,只提當年勇或是前世繡的人,在尋求老後生命階段的存在感與認同,也或許是未能重建個人在此生命階段的重心,需要外界更多的關注與理解。不同生命階段具有角色差異,角色轉換間或許不全是個人選擇,但持續在角色階段間開展人生意義正是生命的價值。透過當年勇與前世繡雖然肯定了過去的自己,但徹底放下過往光環而開創新人生意義,才是生命持續前行的動力。「好漢不提當年勇,梅花不提前世繡」是種提醒,也是種期勉;活在當下感受生命與生活的一切,放下執念與過去的眷戀,為自己找出前行的意義,活出一個具有色彩的明日,年齡增長或人生階段角色轉換的違和感便會不經意的消逝,無須仰賴過去光環而自我肯定。(作者為台北市立關渡醫院院長)

澳門稱台芒果檢出新冠陽性 吳釗燮酸:怎找到芒果鼻子做PCR
澳門特別行政區政府市政署離島綜合服務中心6月30日發文,台灣芒果檢出新冠肺炎病毒核酸陽性,這批100公斤台灣芒果全數銷毀。對此外交部長吳釗燮今發推特表示,這是中國的經濟脅迫,吳釗燮也嘲諷問「是怎麽找到芒果的鼻子去做PCR測試?」吳釗燮推文指出,繼對台灣的魚類產品出手後,中國對台灣的芒果驗出了新冠肺炎陽性,這是中國對台灣的經濟脅迫,而且這次脅迫的藉口很怪異,對於想要偽裝成不是脅迫也毫不具說服力。吳釗燮也嘲諷表示,想知道到底中國是怎麼找到芒果的鼻子來測PCR?This is economic coercion in peculiar & unconvincing disguise. 突破性感染代表疫苗失效嗎?需要無止盡接種新冠疫苗加強劑?
昨天(2022-4-30)看到一篇才剛發表在新英格蘭醫學期刊的編輯評論,心中頗有感慨,所以才決定寫這篇文章來抒發。感慨,是因為看到評論裡的這句話:【關於 Covid-19 疫苗,最令人失望的錯誤是將疫苗接種後的輕度疾病或無症狀感染標記為「突破」。 ……「突破」一詞意味著失敗,創造了不切實際的期望,並導致對該病毒採取零容忍策略。】這一年多來,每次同鄉或親友聚會,新冠病毒一定會有一席之地,而為了要讓大家了解人類和病毒之間的對抗關係,我都會說「突破感染」是個完全錯誤的名詞,根本就不應該被創造出來。我會告訴他們,我們在做疫苗實驗時,會用不同劑量的病毒來挑戰打過疫苗的老鼠。在低劑量病毒的情況下,10隻老鼠全存活;在中劑量病毒的情況下,10隻老鼠死了5隻;而在高劑量病毒的情況下,10隻老鼠全死了。也就是說,只要病毒數量夠高,沒有任何疫苗是不會被「突破」的。不管是打疫苗,還是自然感染,我們所獲得的免疫力(抗體和記憶細胞)是會因人而異,而免疫力高的人可以應付高劑量的病毒,免疫力低的人就只能應付低劑量的病毒。還有,眾所周知,對抗病毒的免疫力是會隨時間而下降的。所以,打過疫苗的人再被感染,是一點都不值得大驚小怪。所謂的「完整疫苗」,不管是兩劑、三劑、甚至四劑,如果打過的人還是被病毒感染,那原因之一是他所接觸到的病毒量是高過於他的免疫力所能應付的限度,原因之二是他所接觸到的病毒是變種,例如omicron。不管是什麼原因,對病毒而言,沒有什麼疫苗是「完整」的。去年4月,台灣出現第一個所謂的「突破感染」案例,而關鍵評論就立刻邀請我寫了一篇評論,請看機師接種過一劑AZ疫苗仍染疫,這代表疫苗失效了嗎?。其中的一段是:【「百分之百有效」是大家喜歡聽到的,而也就因為如此,有些醫藥廣告,包括出自中醫師和西醫師的,就會聲稱他們能百分之百治愈病患(請看新冠肺炎治癒率100%的名中醫)。但是,除了百分之百不可能之外,沒有什麼預防或治療的效果會是百分之百的,因為生物個體間本來就不可能會是百分之百相同,而有些不相同的地方就會影響到個體對疫苗的反應程度。好消息是,縱然是出現「疫苗突破」,完成兩劑疫苗接種的人所出現的症狀,相較於沒有接種疫苗的人所出現的症狀,還是輕微很多。】大前天發表在新英格蘭醫學期刊的編輯評論是Covid-19 Boosters — Where from Here?(新冠加強劑 – 何去何從?)。作者Paul Offit是一位兒科醫生,專攻傳染病、免疫學和病毒學。他是賓州大學兒科和疫苗學教授,也是費城兒童醫院疫苗教育中心主任。他是輪狀病毒疫苗的共同發明人。這篇評論總共有八段,而前五段是在講所謂的「完整接種疫苗」是如何從原來的兩劑,演變成三劑、四劑,甚至是未來的N劑。剩下的三段則是在講疫苗加強劑要何去何從,我把這三段翻譯如下:人們現在對「完整接種疫苗」是什麼意思感到困惑。 這很容易理解是如何發生的。 可以說,關於 Covid-19 疫苗,最令人失望的錯誤是將疫苗接種後的輕度疾病或無症狀感染標記為「突破」。 與所有粘膜疫苗一樣,目標是預防嚴重疾病——讓人們遠離醫院、重症監護室和太平間。 「突破」一詞意味著失敗,創造了不切實際的期望,並導致對該病毒採取零容忍策略。 如果我們要從大流行(pandemic)轉變為地方病(endemic),在某個時候,我們將不得不接受疫苗接種或自然感染或兩者的結合不能提供針對輕度疾病的長期保護。此外,由於加強劑並非沒有風險,我們需要清楚分辨哪些群體最受益。 例如,16 至 29 歲的男童和男人患 mRNA 疫苗引起的心肌炎的風險增加。 所有年齡組都面臨「原始抗原罪」(original antigenic sin)這一理論性問題的風險——由於免疫系統鎖定了原始免疫原,因此對新免疫原的反應能力下降。 這種現象的一個例子可以在一項對非人類靈長類動物的研究中找到。該研究表明,與使用原始病毒株來進行加強相比,使用 omicron 特異性變種來加強,不會產生更高水平的 omicron 特異性中和抗體。 這個潛在的問題可能會限制我們對新變種做出反應的能力。現在 CDC 有責任確定誰從加強劑量中受益最多,並教育公眾了解粘膜疫苗的局限性。 否則,對輕度或無症狀感染的零容忍策略(只能通過頻繁的加強劑來實施)將繼續誤導公眾關於 Covid-19 疫苗能做什麼和不能做什麼。原文:突破感染,加強劑,何去何從

醫病平台/ 除了自費的昂貴免疫療法藥物,是否有第二種經濟上可負擔的選項?
【編者按】本週主題是「昂貴藥物面面觀」。隨著醫療科技的發達,人類戰勝疾病的機會激增,帶來絕症病人新希望,但也同時引起空前未有的新問題。一位最能了解病人與家屬因為醫療帶來經濟壓力的社工師,寫出自己以家屬的經驗以及專業上看到病人與家屬所面臨的困境。一位血液腫瘤專科醫師寫出面對病人與家屬的兩難:一方面殫思竭慮盡其所能地思考如何治療,一方面卻不忘治療可能帶給對方難以面對的經濟困境。一位介入國內新藥通過程序多年的資深專業藥師,分享政府在通過新藥上市的過程,如何戒慎恐懼面面俱到的用心。希望醫病雙方珍惜善用資源,並杜絕濫用昂貴藥物的治療。【延伸閱讀:醫病平台/自費醫材的昂貴藥物治療,是病人的希望還是負擔?】【延伸閱讀:昂貴的新藥是帶來希望或造成負擔?望醫病雙方善用資源,杜絕濫用昂貴藥物的治療】半年前,我的門診來了一位中年男性,夫妻一起進來我的診間,想要尋求癌症治療的第二意見。這位先生罹患了第四期的肺癌,在前一家醫院經過了數個月的治療後,效果很好,癌症病情控制的很穩定,只是最近一次的追蹤檢查發現癌症又有蠢動的現象,他的主治醫師建議他需要再度開始治療。乍聽之下,這個診療的過程十分合理,他的醫師也把他的病情控制的很好,那他為什麼想到另一家醫院諮詢治療的第二意見呢?細細探究之下,這位先生是一位受雇的勞工,負責家中主要的經濟來源,前一階段的治療,他的主治醫師建議他除了標準的化學療法外,應該加上自費的免疫療法藥物,用以增加治療的效果。他與妻子討論之後,接受了主治醫師的建議,也順利的達到了癌症控制的目的。只是這階段的治療,已經將他幾十萬元的積蓄花費殆盡,這次他的主治醫師仍然要他再繼續自掏腰包接受免疫療法藥物的治療,但是他已無力負擔,從他滿是無奈的語氣之中,可以理解爲什麼他選擇諮詢第二家醫院的醫師,尋求除了自費外,是否有第二種經濟上可負擔的選項。在癌症的領域照顧病人超過二十年,猶記得肺癌的第一個口服標靶藥物艾瑞莎(IRESSA®)問世時,在健保尚未將其納入給付的年代,病人一個月需要掏出近十萬元來取得藥物,一天一錠,就像高血壓或是糖尿病,看不到治療的盡頭。如果治療效果很好,就需要每個月不斷的支出這筆費用,如果療效不佳,煩惱的將是下一個治療在哪裡?癌症可以控制嗎?要接受化學療法嗎?當時醫院的同事都認為,病人當下若不用面臨生命的困境,也要面對經濟的困境。現在艾瑞莎早已納入健保的給付,甚至第二代與第三代的口服標靶藥物也同樣有條件的納入給付的範圍,免除了很多罹病病人家庭的經濟困境。然而免疫療法的問世,再度讓病人面臨生命與經濟的兩難。若病人的疾病不在健保有條件的給付範圍內,一次治療負擔的費用將超過15萬元,四次治療就要60萬元,這還只是基本的療程。從藥物發展與疾病治療的歷史看來,藥物只會愈來愈高價,經濟的困境仍然持續。2017年7月,美國食品藥物管理局(FDA)核准了諾華藥廠的CAR-T治療,這是全世界第一個採用細胞基因治療癌症的產品,定價一個療程47萬5千美元,這還只是藥物的費用,若加上醫師、護理、住院等費用,預計超過50萬美元,折合新台幣1500萬元。2019年5月,美國食品藥物管理局再度核准諾華的一款基因療法ZOLGENSMA®新藥,用以治療脊髓性肌肉萎縮症這個罕見疾病,這種一次性的基因療法費用竟高達212萬5千美元,折合新台幣約6375萬元,成為世界有史以來最昂貴的藥物。藥價不斷創歷史新高,令人憂慮,但是藥價只是醫療的一環。從人均健康的支出來看醫療花費,也能窺見一二。世界銀行(The World Bank)根據世界衛生組織(WHO: World Health Organization)全球健康支出資料庫(GHED: Global Health Expenditure Database)的統計(https://data.worldbank.org/indicator/SH.XPD.CHEX.PC.CD?end=2018&start=2000&view=chart),全球人均健康支出從公元2000年的480美元逐年增加至2018年的1110美元,十九年中增加超過一倍。而從台灣衛生福利部於2021年1月發布的國民醫療保健支出年報得知(https://dep.mohw.gov.tw/dos/lp-5071-113.html),台灣人均健康支出也從2000年的741美元(台幣23143元)逐年增加至2019年的1575美元(台幣49011元),同樣的在二十年中也超過一倍。在這份衛福部的官方報告裡,我們發現藥物的支出二十年來每年都約佔總支出的20%,這表示不是只有藥物的價錢節節攀升,醫療中的其他支出也同樣一年高過一年。再看全世界醫療花費最高的美國,在癌症治療的現況。美國醫學協會(AMA: American Medical Association)的官方網路開放醫學期刊(JAMA Network Open)於2021年10月6號刊登了賓州癌症研究所(Penn State Cancer Institute)放射腫瘤科Zaorsky醫師的研究(doi:10.1001/jamanetworkopen.2021.27784),從MarketScan的資料庫與美國國家癌症研究所年度報告(SEER: Surveillance, Epidemiology, and End Results)中發現,在2018一整年中小於65歲的私人保險病人同時罹患前15種常見癌症的人數共402,115人,推估癌症相關治療的支出將達到1562億美元,平均每個癌症病人當年度將花費388,446美元在癌症治療上,折合台幣將達到1165萬元。所有醫療的進步與發展,目的都是為了讓病人可以從疾病中康復。若病人的疾病是不治之症,也希望藉由現代的醫療技術,緩解病痛,延長有品質的生活,進一步維持生命的尊嚴,然而,高價的醫療是不是一定都朝向醫療初衷的方向前進,值得我們反覆思考?現代醫學已經發展成一門十分複雜的科學,醫療工作者今日面臨的責任只有比過去更重大。記得小時候常見的內兒科診所、甚至內外科通才的診所,如今已不易見到,取而代之的是極度專業分工的眼科、耳鼻喉科、皮膚科、小兒科等等。試想,在這些專業科別之間,就算是醫師,也極為可能不熟悉非自己領域的知識,更遑論是一般人。所以我們或可想像,醫療知識在病人與醫師之間的不對等,差距一定更是巨大。從不對等的基礎出發,顯而易見的,醫療工作者的責任之一就是要在浩瀚如海的醫學資料與證據中,去蕪存菁,替病人與其家人理出一份頭緒,經過討論,採用對病人最具經濟價值的方式,達到病人最想要的結果。對於可以治癒的疾病,當全力以赴;而面對不幸的不治之症,溫柔誠實的告知,盡力維護病人的尊嚴。癌症病人面臨的醫療困境,每日都在上演,不僅在台灣,美國也是。藥物的發展,的確延長了癌症病人的生命,甚至將不可治癒的疾病推進至有機會治癒的境地,令人驚嘆。只是在現實世界裡,當病人面臨重病帶來的膽戰心驚外、負擔不起的昂貴藥物理當不該再成為病人心頭的另一個痛,身為醫療的專業工作者,如何協助病人找到最適合而可負擔的治療方案,維護病人的利益,是迫在眉睫的大事。位於美國田納西州曼非斯的St. Jude兒童研究醫院,是一所以治療兒童癌症聞名全世界的機構,醫院的創辦人是一位美國家喻戶曉的喜劇演員Danny Thomas。他想像中理想的醫院是一所為了「治療所有孩童,無關乎其種族、膚色、信仰或家庭財力;成為用研究照亮黑暗的機構(Entertainer Danny Thomas envisioned a hospital that would treat children regardless of race, color, creed or their family’s ability to pay. A facility where research would shine light into the darkness.)」。他曾說過:「世界上有兩種人——贈與者及接受者,有時候接受者能吃得比較豐盛,但贈與者永遠睡得更加安穩。(There are two kinds of people in this world: The givers and the takers. The takers sometimes eat better, but the givers always sleep better.)」他也說:「最棒的報答就是你曾協助挽救性命的孩子臉上綻放的笑容。(The greatest reward is the smiles on the faces of the children whose lives you’ve helped save.)」這間醫院在這樣的理念下走向世界頂尖的地位,也期許未來,希望不要太久,台灣能處處充滿這樣推動研究與病人福祉的機構。

為手術爭取更多時間的腦部冷卻技術 其實源自於嘗試大腦移植的外科醫師之手
2015年,義大利神經外科醫生卡納維羅(Sergio Canavero)表示自己將會是首位做到「大腦移植」的醫生。事實上,他不是第一個有這樣想法並致力落實執行的人,卡納維羅的偶像──羅伯.懷特醫生也說過同樣的話。這是一本引人入勝的傳記和醫學發展史,探究羅伯.懷特(Robert J. White,1926-2010)這位美國最知名、最傑出也是最古怪的外科醫生如何追尋頭顱移植的醫學技術,將一個人的靈魂移植到另一個人的身體。這聽來簡直是天方夜譚,或只會出現在科幻小說。但在真實世界可以怎麼樣辦到呢?懷特的博士論文研究並未從加上第二顆頭著手,而是專注在腦半球,更精確來說,是移除動物的一側腦半球,然後觀察結果。這類手術稱為大腦半球切除術(hemispherectomy),自一九二八年起,偶爾在人體上實施,通常應用在癌症和腫瘤的特例,或治療嚴重癲癎。結果好壞參半:有時候病人會完全康復;有些時候會帶來不尋常的後果,最常見的是失去語言能力或局部麻痺。即使到現在,我們仍不清楚為什麼會這樣,雖然可能的原因不一,或許與神經可塑性(大腦的適應能力)及患者年齡(愈年輕狀況愈好)有關。在某些病例只把左右大腦半球之間的連結切斷,而非完全移除某側大腦半球,一般是為了讓無法以藥物治療的癲癇停止發作。這種方式的復原率往往還更好,雖然會有輕微程度的不適……然而在極少數的情形下,會現奇怪的症狀,包括所謂的「他人之手症候群」(alien hand syndrome),這些人的手掌或手臂(通常是右手)似乎會按照自己的意願行動……在某些案例中還會打主人或掐脖子。換句話說,腦很奇怪又奇妙(有時令人毛骨悚然),我們對腦所知甚少。懷特想知道,到底需要多少分量的腦就夠用。懷特的團隊將兔子麻醉,後來是狗,好移除牠們的一側大腦半球,有時是右腦,有時是左腦。在大多數情況下,這些動物會恢復,帶著半顆腦過大致正常的生活,雖然有些小麻煩,但大多只是行動不順的問題。生命的火花似乎存在於剩餘半球的意識裡,仍然裹在動物的頭骨中。不過,梅約診所同時也是創傷醫院,而懷特是腦外科醫生。他一次又一次看著年輕的意外受害者,他們的頭骨遭重壓或刺穿,陷入昏迷,再也沒有醒來。如果你能夠只靠半顆腦活下去,那麼,你要怎麼解釋這些災難?懷特曾經用手拿著一顆孩童的腦。他曾經聽到家長帶著期盼,乞求答案問道:他們的寶貝何時會好起來?在那些憂心忡忡的問診當中,懷特只能緊閉薄脣;其實他認為復原無望。但是,你會告訴才剛陷入悲傷狀態的媽媽和爸爸嗎?或者,你就是等待,讓時間在接下來的幾天或幾週揭曉?有時候,懷特會和這些家長一起祈禱。手術後,他會到附近的天主教堂停留,也在那裡禱告。然而,對於神經系統受損的病人來說,他知道祈禱無法改變他們的結局。這種矛盾的情況使他苦惱:如果大腦半球切除術證實人可以只靠半個大腦活下來,那麼大腦受創後妨礙意識恢復的大敵似乎不是創傷本身,而是別的因素,是手術後才出現的東西。懷特懷疑罪魁禍首應該是脊髓組織的腫脹,這會讓創傷後通往腦部的血流受阻,但是他沒有把握如何精確地中斷這種效應。而答案將從不尋常之處冒出來─又是與狗有關。懷特來到梅約診所的好幾年前,診所就一直在研究分離腦部的主題。他們的目的在於分離出狗的腦(也就是把它移出頭顱,並維持活力),希望能創造出所謂的「模型」。分離出來的器官讓研究人員可以了解代謝率(器官消耗能量的速率)、血液如何在裡頭循環,以及其他因素,而不需要去分辨器官和身體的差別(身體有自己的循環模式和代謝情形)。如果外科醫生沒有從分離研究(並外推到人類)而來的資訊,可能會誤判某種器官維持穩定所需的血液量,在複雜的手術中,沒有餘地容許犯這類錯誤。懷特在一九五五年來到診所時,他的新同事還沒成功完成分離,他們每次嘗試時都會意外壓壞、割傷或損害組織。懷特已經以手術精湛聞名,他的大腦半球切除術堪稱藝術傑作。他很快就加入獸醫暨資深生理學家大衛.唐諾(David E. 初臨失智/長輩失智症狀嚴重嗎?6原則,教你如何長期追蹤、判斷嚴重度
無論失智症或中風的病患,目前是在醫院還是家中,要觀察患者的認知功能缺陷或精神症狀(以下簡稱症狀),都是一件不容易的事。患者在家中時,可能因為家屬沒有醫療相關背景,每次症狀出現時,只能專注在如何解決問題,經常是無暇,也不知道該如何觀察起。而在醫院的臨床工作者,大部分只能根據家屬的口述,去了解病人目前的狀況,此時,「時好時壞」是大家描述症狀時常見的形容詞。其實,我們可以透過六個不同的面向,去量化時好時壞的問題(4WHF):1、Where觀察症狀出現的所在地方,我舉個比較容易理解的例子:是在家中或是外出時?或特別是在醫院。因為如果在醫院就診,可能代表病患比較願意外出,家人也比較有理由強迫他有出門的機會。在家中,則可能認不得人或叫錯名字,外出則什麼人都清楚誰是誰。2、What觀察有哪些症狀出現,現在的精神症狀如何?是否會算數?會認人嗎? 曉得物品的名稱嗎? 現在何年何月何日等等,以上都是有可能出現的症狀,這部分是家屬比較容易注意到的問題。3、When觀察在哪些時間會出現症狀,是在白天?還是半夜、睡醒時?或是沒睡覺時,也會出現。有許多症狀,可能會伴隨著睡覺出現,所以可以注意個案是否有這方面的傾向。4、Who觀察誰會遭遇到這些症狀,同一個症狀,個案可能會對不同人做不同的反應喔!例如:當媳婦問他問題,個案可能會說不知道;但孫子問時,個案就很積極的去想答案。也有對家人會出現某些問題,但對醫師就不會有問題。5、How觀察症狀如何發生,觀察這件事情的來龍去脈。例如:當家人發現個案迷路回家之後,訊問清楚他當時的狀況,後續的情形,以及最後如何回到家。6、Frequency觀察症狀出現的頻率,將有助於照顧者和醫療人員判斷目前個案的嚴重度。過去曾經詢問一位重度失智症病患的兒子,了解病人在這一年能夠認得他幾次,家屬表示只有一次正確叫出兒子,其他都是叫弟弟,其他認識的人名等,這就可以顯示出個案罹病相當嚴重。評估頻率也需要方法●首先,觀察較少發生的症狀。如果正確反應的次數少(例如:認得家人的次數比較少),就記錄正確反應 ; 錯誤反應的次數少(例如:認錯家人的次數少),就記錄錯誤反應。其次,需要設定日期區間,例如在一個月裡或一週裡發生幾次。●最後,需要比較頻率。這部分特別有助於評估病患的嚴重度,譬如在106年的5-6月間,病人出現了3次不認得人;而在107年同期,一週就出現5次不認得人的情形,這樣就可以觀察到個案的衰退傾向。衡量頻率也需要注意,不需要每個月或是每週去看,因為如果是疾病造成的症狀,不會在短時間內會有太大的改變(庫賈氏症則例外)。「時好時壞」可以不再是抽象的形容詞,而是可以很具體的,透過分析、描述這些症狀,以便讓其他人更容易理解患者目前的病程,也較容易想出對應的方法。※溫馨小提醒:1、當認知或精神症狀出現時,別單純的處理症狀。2、以4WHF方法確立時好時壞的症狀。3、長期追蹤症狀頻率,有助於判斷病患症狀的嚴重程度。(本文獲「「黃耀庭臨床心理師」授權轉載,原文:描述失智症狀可以別再說「時好時壞」)

醫病平台/醫院裡不可或缺的存在! 美國麻醉科醫師照顧新冠肺炎病人的內心世界

I just didn’t realize how essential I was, and never would have described myself that way, until the coronavirus pandemic. Millions cannot go to work due to shelter-at-home rules, but I have to: I’m an anesthesiologist. As I pass through the checkpoint to enter the hospital with other essential workers, I am reminded of what my single-parent mother instilled in my sister and me when we were little. While we sat in the dark, unable to pay the electricity bill, she said: “If you don’t want to live like this, get an education.”We were never homeless or hungry, but the apartment was also not well maintained, with holes in the plaster, and bars on the back windows after the woman who lived on the floor below us was raped. My mother never complained to the landlord – she taught us “Never ask for help, the world is not here to help you.” If I didn’t have enough money for a packet of bus tokens, I was not to ask anyone for a handout. At a young age, I stuffed envelopes and babysat to earn money.On the other hand, I had an unbelievably rich childhood – my mother was a costume designer and I sawThe Misanthrope and Endgame from the wings of the theater during dress rehearsals. I also loved the library. Even though I was afraid of librarians, the library was a safe place to go after school, and safety, besides a livable wage, was another lesson drilled into me. At home, my sister and I played a game with our library books, placing them along the floor and pretending that if you stepped off them, you sank into a watery abyss. But we knew, or at least we believed, the abyss wasn’t real.When my mom said get an education, I listened. I went to an outstanding public high school – a magnet school that drew the brightest kids from all over Philadelphia. I attended college on full financial aid and majored in biochemistry. I planned to get a Ph.D. but during a summer work-study job, I met a post-doc – someone who already had achieved what I was convinced was the pinnacle of education. He told me he didn’t have a job the next year. This blew my mind. In that moment, I knew I needed to seek a new direction, something where I could always have a job.Later that year, I had an epiphany while walking outside my dorm at a site rumored to be a film location for Love (‘means never having to say you’re sorry’) Story. Everyone gets sick, even Ali MacGraw’s character. If I became a physician, I’d always have a job. Eureka! Of course, in medical school interviews, I couldn’t come clean about my reasons. As much as I loved science, I knew I had chosen this path because I felt I would always be employed.Yet, at medical school, something changed. Love happened in an unlikely setting. I felt strangely fatigued during my anesthesiology elective, but enjoyed the people and culture of this hidden part of medicine. Delirious and febrile from mononucleosis-induced hepatitis, which I did not initially know I had, I fell deeply in love with the quirky, fulfilling specialty of anesthesiology.Fast forward through decades of academic anesthesiology practice – and, despite risks to mental health from stress and the myriad ways in which a minor disability could render the anesthesiologist useless, I had (you’d be proud, mom!) a safe and secure job every day of my life. But the job and its safety have changed.Donning enhanced personal protective equipment, I check the barriers protecting me as an anesthesiologist during and after the airway procedures I will perform - procedures which make the virus even more contagious. I mouth breathe, pant really, in my N95 mask and hood, and, encumbered by impermeable layers and double gloves, move my limbs slowly as if I was in a phony moon landing scenario. I loudly ask the nurse to leave the room; sometimes I have to shout to be heard by the respiratory therapist. I look down at my frightened or too-far-gone-to-be-frightened patient. I’m the last person they will see before I push sedatives and place a plastic tube between their vocal cords. “I’m giving you medicine to make you very sleepy,” I say. “Medicine to put in a breathing tube to help you breathe. We will take good care of you.” It sounds harsh because I have to speak so loudly.I understand, retrospectively, I was an essential worker through HIV/AIDS, SARS, MERS and Ebola. But because of where I live, my limited exposure, and how these diseases are transmitted, I never felt the fear that is my steady companion now.There is a longstanding analogy involving airline pilots and anesthesiologists, which compares take-off, flight, and landing to stages of an anesthetic. There are benefits to the analogy – it’s why simulation and communication training, developed for pilots, is now standard in anesthesiology education. There is a big difference, though: if the plane goes down, the pilot dies too; but if the anesthetic goes awry, only the patient dies.As another overhead code call to the emergency room blares, and I, on the COVID airway team, grab equipment we pack in wheeled suitcases, respond to texts from team members, and don my N95 and eye protection before hitting the ER – the rest of the equipment will need to be donned and checked outside the patient bay – I realize, almost cellularly, that things are different now. With COVID-19, I could die. Or I could cause my loved ones to die if I bring the virus home. It would be like a slow but inevitable plane crash. A younger colleague (and now they are all younger) came up to me one pandemic day. He said his heart rate was 130 but he otherwise felt fine. His temperature, heart rhythm, oxygen saturation, and blood pressure were fine. I told him, “It’s okay, go home, drink some water and relax. I’ll do your case.” He went home and his heart rate normalized. It was anxiety, not virus. This level of anxiety would have never happened to him pre-COVID. It’s not just the virus that threatens all of us, it’s also the fear.Until now, I hadn’t truly thought about the danger of being an essential worker. But my mom equipped me to deal with enormous stress. Taught me I was loved. And maybe, in watching her go it alone for so many years, taught me to live a life different than hers. She taught me to be a lifelong learner, and what I learned from my colleague, to seek help from others, is one of the most valuable lessons of my life. It’s okay to be vulnerable, it’s okay to tell the teacher selling bus tokens you don’t have enough money.What I learned in the pandemic is this: we are all in this together. We are all needed. None of us is alone in this world. 【文、圖/摘自商周出版《手機、咖啡、情緒的化學效應》,作者阮津玫】久坐和抽菸一樣致命?大家正悠哉舒服地坐著看書嗎?趕快站起來!因為:「久坐和抽菸一樣致命。」「凡久坐,必早死。」「缺少運動的德國死亡人數是抽菸者的兩倍。」我的工作經常需要出差,忙東忙西。相反的,宅在家裡的時候,我的屁股總是黏在椅子上,很少起身走動。最近我花很多心思與時間撰寫這本書,坐在電腦前的時間更變得有增無減。只要待在家裡工作,我會和馬修同時起床,然後立刻打開電腦,「簡短地」回覆一些電子郵件。馬修早上七點去上班,傍晚六點回到家。對我而言,這段時間「颼」一聲就過了,彷彿僅僅一個小時。我可能已經整整工作了十一個小時,身上卻還穿著睡衣。因此,我知道自己應該多站起來活動活動筋骨。因為科學研究已經證實:久坐和抽菸一樣致命。奉勸老菸槍們不要坐著抽菸,還是一邊吞雲吐霧一邊散步吧!「科學證實之事」如果聽起來很瘋狂,那麼或許它的事實並不瘋狂,或者根本無法百分之一百通過科學的檢驗。以「久坐和吸菸一樣致命」這句話為例,它的確透露出一部分真理,不過卻也有些浮誇。先討論這句話糟糕的地方。在科學領域裡,「久坐的生活型態」(sedentary lifestyle)經常與心血管疾病、肥胖、第二型糖尿病、癌症及憂鬱症劃上關聯。我真想知道,人類祖先一天裡有多少時間是坐著的?也想了解,人類祖先是不是一逮到機會就想坐在地上或是石頭上,因此才發明了椅子?還是說,久坐根本違反人類生物特徵,只是文明的(危險)產物?專家們傾向於支持第二種觀點。巴德萊醫師在〈久坐等於新的吸菸〉(Sitting is the new smoking: Where do we stand?)一文中提及:外星訪客對於現代人類的生活型態一定倍感驚訝,尤其是現代人類的身體活動量。在過去六百萬年的歷史當中,人類忙著狩獵及農耕。現在呢?大家待在舒服的辦公室室內空間裡,坐在舒服的沙發上,看著電腦螢幕。完全不想用腳走路。上下樓層,寧願等電梯。穿梭世界五大洲,一定選擇搭飛機。另一方面,外星訪客也會心存懷疑:同樣的這批現代地球人,為什麼不論颳風下雨都熱衷慢跑?他們為什麼願意花錢上健身房,努力費勁地做舉重鍛鍊呢?或是寧願在跑步機上跑得滿頭大汗氣喘吁吁?唉,我也繳了健身房年費,只不過不常去罷了。我猜,很多健身房會員都和我一樣,僅僅掛名擔任「乾股東」。事實的確如此,越來越多的現代人淪為慢性病(Noncommunicable Diseases,簡稱NCD)患者。慢性病並不會傳染,卻像「現代瘟疫」一樣大肆流行。這些疾病進程緩慢,屬於長期的慢性疾病。四大慢性病指的是:心血管疾病(例如心肌梗塞或中風)、癌症、慢性呼吸系統疾病以及第二型糖尿病。根據世界衛生組織的統計:全球七成一的死因來自於慢性疾病;在三十歲至六十九歲的年齡群當中,全球每年因慢性病之死亡人數高達一千五百萬人。寫這些告訴大家,不是為了破壞大家的好心情,而是為了告訴大家:大部分的這些慢性病是可以預防的。抽菸、飲酒過量、飲食失衡以及缺乏運動,就是最嚴重的風險因子。逆轉命運,操之在己啊!眾人早就明瞭:運動有益健康,不運動對身體不好。幾乎動也不動的「沙發馬鈴薯」到處都是。這究竟有多危險呢?簡化的科學有什麼不妥?在網路上搜尋「久坐和抽菸一樣致命」,相關報導堆積如山。其中經常提到,即使規律運動也無法抵銷久坐的負面影響。(老天爺啊!既然如此,我也甭去健身房了。我每天幾乎都坐著,身體早就完蛋了!)重要的是:切勿連續坐太久。每個小時至少要站起來動一動,或者站著打電腦工作。不然的話,從健身房或慢跑辛辛苦苦得來的健康促進效果會被扣光光。如此看來,久坐不僅僅是缺少健康的運動,更對健康有害。這類說法究竟有何不妥呢?我很樂意回答這個問題,但請大家瞭解:科學研究給的答案通常都又臭又長,而且不一定正確。我們會認為,科學旨於陳述事實,必須清晰明確。其實,並不盡然如此!科學家傾向於運用數字及測量值來呈現研究結果,但解讀數據卻是一門相當複雜的學問,未必每次皆可解釋清楚。某些情況下,必須透過實驗加以推測及驗證。不過,推測並不等同於事實,最多只是「有科學憑據的推測」。基本上,眾人傾向於認為:「經過科學證實的論點等於就是事實。」但被證實之事並不等同於事實,只不過是合理的推測罷了。其中部分觀點可能被視為「真」,但之後也可能被其他研究推翻。想學習科學思考的方法嗎?那麼,請你不要輕信簡化過的答案。舉個例子:我邀請同事來家裡晚餐。保羅是第一次來。我既不清楚他喜歡的口味,也不知道他的飯量。那天晚上的主菜是備受眾人青睞的義大利燉飯。大家吃得盤底朝天,頗符合我的期待。雖然保羅也一直讚美桌上的食物,但只有他一個人沒吃完。疑問:為什麼保羅沒把燉飯吃完?可能的回答包括:他不喜歡燉飯。或者,他肚子還不餓。或者,他飯量小。或者,他正在減肥。大家可能會給出這些簡單明瞭的答案,對不對?唉,如果問科學家這個問題,他們的回答可能是如此:和當晚其他同事的飯量相比,保羅的飯量比較小。保羅可能吃得比一般人少,或者其他同事都是「大胃王」。一直以來,其他同事們享受的燉飯量通常都比較多,只有保羅一個人吃不完。綜上所述,明顯的證據顯示:此事起因於保羅異於常人的飲食行為。很多不同的原因都可能引起保羅的食量低於平均。其中一項可能原因是:保羅對於飢餓的感知能力低於一般人。不過,有關這項原因,目前尚待釐清。另外,也可能是因為保羅湊巧吃了太多午餐,或他習慣只吃少量的晚餐。另一個原因可能與保羅的口味喜好有關。他可能不喜歡義大利燉飯的口味,因此一律拒絕品嚐義大利燉飯,或他特別討厭玉米義大利燉飯。不過,為什麼他還讚美我做的燉飯呢?基於以往累積的觀察經驗,我們知道:這類的恭維只不過為了討好主人,強化正向的社會互動關係,未必符合事實。或者這是社會標籤。目前缺乏有關保羅平常飲食習慣的資料,也不瞭解他的社會行為,因此務必謹慎地採用社會互動觀點的詮釋。保羅當晚的行為成因亦可能來自於數項原因之加成效果,但目前無法確定有哪些相關的影響因素。必須進行後續研究,加以釐清。大家還清醒嗎?還是已經睡著,自動停止思考了呢?科學期刊裡的文章大約就是這種形式。真的!我發誓,沒騙你們!但原則上,報章雜誌裡的科普文章就不是這種調調。當然,這得歸功於科學新聞記者的妙筆生花。不過在詮釋科學研究結果時,科學記者們也可能過於簡化或取材錯誤。問題在於,我們很難只憑科普報導來判斷真正的研究結果。當然,我們可去查閱原始文獻。但科學文獻裡充斥著專業術語,根本就艱澀難懂!而且科學文獻乃以專業語言為基礎,充滿鉅細靡遺又層層分化的文字堆疊。你我想從其中找出重點,簡直比登天還難。相較之下,前面那段有關「保羅與燉飯」的專家說法還算淺顯易懂。但請問大家,有誰能彙整出那段內容的重點呢?不容易吧!對不對?科學文獻的閱讀難度高,一般人無法駕馭。加上部分科研結果並未向社會大眾公開,必須付費才可取得閱讀權限。在種種原因背景之下,媒體開始促成科學與公眾之間的交流。透過這樣的解釋,大家能夠瞭解優質科學媒體的重要了吧?但記者們也心知肚明,媒體喜歡簡潔的答案以及俐落的標題。如果能夠列出既簡單又戲劇化的大標題,更讚!就是因為這些原因,坊間才出現了例如「久坐等於新的吸菸」等駭人聽聞的字句。

餐飲業得認清:即便疫情消散,消費者也可能回不來了!
你應該要知道的食事隨著生活型態的轉變,「三餐老是在外」的人口理當越來越多,不過由於新冠肺炎疫情延燒,全球各地有成千上萬的人在家中處於自我健康管理或被迫隔離的狀態,無法外食的狀況也逐漸增加,疫情對我們的生活方式、飲食習慣已經出現了重大的改變。自2020年1月21日台灣發現首起新冠肺炎(COVID-19)確診病例,疫情蔓延了數月,人們開始懼怕病毒在人與人之間快速傳播,因而降低了外出的次數,全國各行各業都受到了不同程度的影響,而首當其衝受影響的產業之一即是非常需要顧客上門消費的「餐飲業」。當你作為餐廳老闆,在「等嘸人」上門的此時,心裡只想著疫情過了,景氣就會復甦、客人就會回流?但你沒注意到的是,消費者此時飲食習慣與行為的改變,可能會讓他們開始重新思考往後的飲食方式。你必須認清的是:消費者可能已經回不去了!覺得外食不安全,「增加自煮頻率」、「在家用餐」成新趨勢根據《食力》「外食族防疫大調查:餐飲業怎麼做才能讓你安心?」網路問卷調查結果顯示,新冠肺炎疫情爆發後,為了避免外食產生無謂的接觸,有34.7%的民眾選擇「增加自煮頻率」,其次則為「改成外帶(19.1%)」。同樣地,全球市調公司尼爾森(Nielsen)2020年3月27日發表針對香港、韓國、馬來西亞、中國、台灣等11個亞洲國家的消費者調查,同樣發現多數民眾「在家中用餐」頻率比在疫情發生前還高。在中國,有86%的民眾優先選擇在家用餐,其次則為香港(77%)、韓國(62%),馬來西亞(62%)和越南(62%)。此外,調查也發現,外帶食物以及外送到府的需求也增加了,尤其是在香港、韓國、泰國等,這些行為對餐廳的服務內容都將產生明顯的影響。隨著生活型態的轉變,「三餐老是在外」的人口理當越來越多,不過由於新冠肺炎「全球大流行」的此時,全球各地有成千上萬的人在家中處於自我健康管理或被迫隔離的狀態,待在家裡的時間也逐漸增加,很明顯疫情對我們的生活方式、飲食習慣已經出現了重大的改變。台灣大學公共衛生學院院長詹長權也表示,疫情衝擊之下,已經沒有「想當然而」的事情,所有短暫的行為都可能變成永久,「創造新常態」已經成為產業和個人要面對的現實。消費者此時的改變,將可能成為常態尼爾森(Nielsen)東南亞區總經理Vaughan Ryan表示,自2020年1月底以來,許多亞洲市場每週民生消費用品的銷售額超過過去平均的20~25%,雖然有搶購囤積的案例發生,但民眾的消費習慣已經演變成「非恐慌式」的搶購,而是轉為「家庭式消費」的趨勢,也就是說,越來越多消費者「選擇在家中利用網路購買商品或食物」。面臨消費行為的變化,緊接著迎面而來的問題是:「什麼時候恢復正常」,而答案則是「可能永遠不會」,為什麼?Ryan指出,「消費者的思維以及行為已經重新定位,這可能會造成長期的後果。」以中國為例,有89%的消費者表示,即便疫情結束,仍會願意在線上購買日用品、鮮食等,新的習慣(在家用餐、外帶、外送)可能會永遠取代舊的習慣(外出用餐)。餐飲業的下一步能夠怎麼做?無論是大型餐廳、小型餐館,在疫情衝擊下來客數快速下降,很少有企業能夠倖免新冠肺炎所附帶的損害,作為餐廳老闆,能夠怎麼做減少損失?1、拓展外送服務,不能用原菜單上架儘管減少外出、居家隔離等原因使得人們在家中的時間變長,但也因此出現了一種新的生活方式,例如線上購物,消費者也迅速地適應了這種狀況以滿足日常的需求。為了因應消費者「在家中用餐」頻率的增加,餐飲業者也將「外送服務」作為支撐業績的手法之一。舉例來說,統一時代百貨台北店3月份的餐飲業績就因為疫情衰退了近1成的業績,因此轉而積極強化外帶及外送服務,目前館內已有16家餐飲品牌與foodpanda外送平台合作。而針對曾有其他業者希望百貨可以讓利、降低抽成,統一時代百貨台北店總經理王寶銘表示,因百貨大多屬「抽成」機制,如果業績低相對的抽成也少,因此會再整體考量,視各別的狀況特別處理。此外,社團法人台灣服務業發展協會總顧問李培芬指出,成本的改變,業者勢必得針對外送平台做出菜單上的改變,此外她也建議「菜單不是越豐富越好」,在現今這個非常時期,為了減少損耗,減去銷售量低的菜色項目,備料不需要太多。2、跨品牌合作策略,滿足消費者的需求!台灣連鎖暨加盟促進協會秘書長劉孆婷也認為,外送的餐廳那麼多,如果業者只是推出平時的餐點,很難脫穎而出,因此可透過品牌跨界合作、或者是商圈合作的方式,用「搭餐」的形式加強餐點優勢。例如寧夏夜市在2019年尾開始與Uber Eats合作,夜市內不同攤商共同「組隊」,可以跨店組成套餐,可以有更多元的選擇,也可以刺激彼此的業績成長。據了解,士林商圈、圓山商圈、公館商圈等店家都可能會納入外送平台的合作範圍。3、開發冷凍商品,開拓「宅商機」除此之外,也有餐廳業者為了因應民眾「自煮頻率」的增加,順勢推出「冷凍料理包」。以和億生活集團旗下的「添好運」為例,就率先全球推出冷凍冷藏食品,包括流沙包、鳳爪、燒賣等,讓消費者在家也能品嘗到米其林餐點,搶攻防疫商機!而涓豆腐、莫凡彼、鬍鬚張、哈肯舖手感烘焙、黑面蔡滷春秋、樂檸漢堡,以及魔法咖哩與Pino Pizzeria Ristorante等8家餐飲業者,也攜手共組「餐飲大聯盟」,結合各家具代表性的人氣冷凍商品,推出「聯名居家套餐7日組」料理組合,更是跨界合作搶奪宅商機的代表。4、精緻高級餐廳可推「家庭組合餐」而理論上,著重於「餐飲體驗」的精緻餐飲(fine dining)類型的餐廳理論上不適合做外送、外帶,但這些餐廳為了生存也開始改變了作法,「Taster 美食加」創辦人高琹雯就以比佛利山莊知名米其林餐廳「Spago」為例,其推出每人59美元、讓客人到店自取的「家庭餐」,一次可享受多種精緻餐點。高琹雯認為,「由於台灣疫情截至3月中仍算平穩,因此截至3月為止部分餐廳相對不受影響,尤其以內需為強的fine dining餐廳更是如此。而國外觀光客占比較大者,或聚餐型餐廳則會影響較大,估計業績會受到3~4成左右的影響。」她也建議,隨著疫情越來越嚴峻,餐廳業者應儘快做出相關的因應措施,因為國外已經有非常多應變的例子,包括外帶、募資、發放優惠券等等,即便台灣還沒走到這一步,我們還是必須超前部署,儘快盤算下一步措施!疫情的延燒,沒有人能夠確定是否僅會持續數個月或甚至一年以上,但可以肯定地說,新冠肺炎已經確實影響了我們的生活方式,消費者飲食習慣的改變,在未來的時代,都將讓餐飲業發生巨大的變化!餐飲業又要如何在疫情過後復甦?你已經不能等待,勢必得順應趨勢、調整策略,才能夠真正度過危機!延伸閱讀▶回不了頭的餐飲戰「疫」!你是起身備戰,或坐等淘汰?▶從餐飲集團到街邊小吃,沒戴口罩不做防疫,那生意就別做了!▶害怕共食與自取,自助餐與合菜餐廳受創最深!▶大裁員,不如共覓生機!抗疫「持久戰」餐飲業如何帶著員工活下去?▶餐飲「外送」搶客潮!會是短期紓困的解藥,還是長期毒藥?▶想靠打折撐過去?餐飲業想活命得有更多創意!參考資料▶Nielsen ,COVID-19 Where consumers are heading?(本文獲「《食力》」授權轉載,原文刊載於此)

加護病房都用溫水洗手?洗手水溫會影響病菌數量嗎
今天看完病人在洗手的時候,發現水特別熱,想說,是不是加護病房的洗手水溫有特別意義呢?這個問題最先想到的就是就是阿長,之後又去查了一下文獻,最後在問一下資深護理人員結果最後的結果是..沒有特別意義那為何會有熱水呢?因為護士常常造洗手,天氣冷,洗冷水手不舒服,所以改用熱水但是如果又太熱的水,手更容易乾燥,所以建議都用溫水。不過的確有部分文獻說,溫的肥皂水或是含有滅菌成分的水可以幫助清潔更多細菌:http://web.archive.org/web/20080325225216/www.cfsan.fda.gov/~dms/a2z-h.htmlHot water that is comfortable for washing hands is not hot enough to kill bacteria. 讀者0952dc利用本網站的「與我聯絡」詢問:教授你好,請問一下關於精油對抗感冒或流感等上呼吸道病菌~目前有無實驗是直接將瓶子拿到鼻子吸入~又或是滴於衛生紙上使之味道自然蒸發於房間中的吸入也同樣有效果呢? 謝謝 我點擊上面那個連結,看到電視新聞裡一名家醫科醫師在談「精油防疫」。她說:「有的精油它是直接就可以阻止我們的病毒」。這個影片在短短兩天裡就有八萬多個點擊,有262個讚,12個爛。影片的下面還有這麼一段話:疫情延燒,防疫物資引發搶購風潮!澳洲一家大學發現,茶樹與尤加利樹精油,可以隔絕9成以上的流感病毒,雖然目前與武漢肺炎沒有直接關係的直接研究,卻意外帶旺精油銷售,精油業者就透露,最近業績確實有增加。可是呢,我到谷歌搜索,又到公共醫學圖書館PubMed搜索,就是搜不到有任何資訊可以證明《澳洲一家大學發現,茶樹與尤加利樹精油,可以隔絕9成以上的流感病毒》。不過,在做谷歌搜索時,我倒是有看到這兩篇文章:No, ‘anti-viral essential oils’ probably won’t stop you getting ill(不,“抗病毒精油”可能不會阻止您生病)。Dr. Pimple Popper shut down beauty guru Michelle Phan’s claim that burning ‘antiviral’ essential oils can kill off viruses before they enter your system(Pimple Popper醫生關閉了美容大師Michelle Phan的主張,即燃燒“抗病毒”精油可以在病毒進入您的系統之前將其殺死)。這兩篇文章都是在講一位擁有兩百多萬粉絲的美容網紅Michelle Phan用推特聲稱《精油可以對抗病毒》,而一位皮膚科醫師Sandra Lee(化名Pimple Popper醫生)立刻回應說:「很抱歉,所謂的《抗病毒精油》根本就不存在」。這位網紅也回應說,她只是分享了一位朋友的東西,而非有意誤導,她下次會小心留意。也就是說,她也承認所謂的《抗病毒精油》根本就不存在。那,到底是什麼原因會讓台灣這位醫師說精油能抗病毒呢?我用她的名字做搜索,這位專家是《台灣芳香醫學醫學會》的理事長。所謂的《芳香醫學》其實是叫做《芳香療法》,而它的英文是aromatherapy。它並不是正統醫學裡的一個分支,而只是一個旁門左道。有關此一療法,信譽卓著的梅友診所有發表What are the benefits of aromatherapy?(什麼是芳香療法的好處?)。它開門見山就說:關於芳香療法有效的研究是有限。約翰霍普金斯大學也有發表一篇Aromatherapy: Do Essential Oils Really Work?(芳香療法:精油真的有效嗎?)。在這兩篇文章裡是完全看不到virus(病毒)或flu(流感)這個字。約翰霍普金斯大學也有發表一篇2019 Novel Coronavirus: Myth vs. Fact(2019新型冠狀病毒:迷思vs事實)。它共列舉了6項迷思,其中一項是:您可以通過使用精油,鹽水,乙醇或其他物質來保護自己免受2019年新型冠狀病毒的侵害。然後它說事實是:這些建議中的任何一項都不能保護您免受冠狀病毒感染,甚至於其中的一些做法可能很危險。哈佛大學也有發表一篇Be careful where you get your news about coronavirus(要小心從哪裡獲得有關冠狀病毒的新聞)。它說:「《牛至油(oregano oil)證明對冠狀病毒有效》是一個沒有根據的說法」。總之,精油能抗病毒是一個完全沒有科學根據的說法。台灣這位醫師的聲稱,唯一的用處就是幫助精油的銷售。如果精油真的能抗病毒,那她為什麼還需要戴著口罩?有關精油的用途與胡扯,請看《精油能影響紅血球排列?》。原文:精油能抗武漢病毒,醫師說的
